Patient Forms

If you are a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival.  Thank you and please contact our office if you have any questions at all.

Payment Options

Patients are expected to pay for our services at the time they are rendered.  Our patients who have dental insurance are expected to pay the amount of their estimate co-pay and deductible at the time of service. If a pre-estimate is not back by the time of service then we will collect a 20% down payment for restorative treatment, 40% down payment for Crowns, and 20% down payment for Endodontic treatment.  This is just a portion towards the treatment and the remaining balance will be due immediately after insurance pays.

Payments can be made with either check, cash, Visa, Mastercard, American Express, and/or Discover. Unpaid balances will be charged a monthly finance charge of 15% after 90 days.

We also offer CareCredit, in which you can apply for online or by telephone. You may choose to pay with CareCredit over 6, 12, or 18 months with no interest.

Optional Payment Terms:
    1. Full Payment: We offer a 10% courtesy discount for all treatment paid in full on or before the day of service. (Does NOT apply to patients with insurance.)
    2. CareCredit: We can offer, upon approval, an interest-free term loan for 6, 12, or 18 months with no down payment.
Please note that there will be a fee for any addition treatment that is not included in the original treatment plan.

In order to best serve our patients, and keep the cost of dental care as low as possible, we try to maintain an efficient appointment system. However, our cost of providing care increases greatly when an appointment is missed or cancelled at the last minute. Our new policy as of August 2010, is that patients who miss or cancel an appointment with less than a 24 hour notice for the second time will be charged a $15 fee. After 3 missed or cancelled appointments with less than a 24 hour notice, it will result in dismissal from our practice.

Dental Insurance

As a courtesy to our insured patients, we submit claims to your insurance company. We will help you to receive your maximum allowed benefits. In order to do this, we need your insurance card with you on your first visit, or whenever there is a change in your dental insurance. The practice will diagnose treatment based on your dental health and not your insurance coverage.

You must realize however that:
1. Your insurance is a contract between you, your employer, and the insurance company. We are not a party to that contract.
2. Dental insurance is not meant to be a pay-all: it is only meant to be an aid. Many routine dental services are not covered by dental insurance at all. If you should have any questions regarding your plan, you should contact your company.

If your insurance has not paid within 90 days of treatment, you will need to make full payment to this office and be reimbursed when your insurance company pays.
